Instructions: This invoice portion can be emailed to the Office of the Vice President below and the hard copy should be mailed with your check with your registration totals and types listed below. Upon receipt of this document and your check you will receive an email confirmation.
Agreement: The teams identified in this document will represent said league in all competition local, state, regional, area, territory, or national in accordance with the rules of the USA Softball CT and the USA Softball. This form along with the associated fees, shall be forwarded to the office of the State Commissioner. All participating league teams must be registered. Registration with USA Softball CT DOES NOT INCLUDE TEAM ACCIDENT OR LIABILITY INSURANCE. Teams carrying insurance offered through the USA Softball ARE NOT COVERED UNTIL the team and/or league is registered with USA Softball CT. USA Softball CT and the USA Softball are not responsible for any injuries or accidents to any league team or league member. By my signature below, I attest that I understand and will abide by, the aforementioned established deadlines and guidelines and rules setforth by USA Softball CT and USA Softball.
